Our mobility and frailty care

As we get older, arthritis, osteoporosis, diabetes, heart issues or elective surgery, like a hip or knee replacement, can all affect our mobility.

Because of this we can feel less confident in our usual daily activities and tasks. Mobility issues can significantly impact the way we wish to live our life, resulting in a loss of control and independence. This can be a worrying time for you and your family.  We can help, with a care plan tailored to your needs.

Helping clients and your loved ones

Supporting the wider family and loved ones throughout your care journey is essential.

Therefore, to improve communications we can include people involved in your care in your “care-circle”.  This is a special feature of our state-of-the-art care management software and is only done at your request.  It allows your loved ones and your wider healthcare team to keep up to date with your care and gives them peace of mind about your progress.
