Our approach to Mobility & Frailty Care

Our Care Professionals provide comprehensive support to people living with mobility or frailty issues.  From companionship, help around the home, support with medication to taking you out and about, we will develop a comprehensive care plan with you to help you live the life you love.

Decreased mobility and frailty can be a consequence of ageing or the result of an illness, operation, accident, or fall. No matter what your situation or needs, our Care Professionals are here to support you with a tailored care plan.

 

 

 

Our Mobility and Frailty Care

As we get older arthritis, osteoporosis, diabetes, heart issues or elective surgery, like a hip or knee replacement can all affect our mobility.  This can make us feel less confident in carrying out our usual day-to-day activities and tasks. Mobility issues can significantly impact the way we wish to live our life, resulting in a loss of control and independence. This can be a worrying time for you and your family.  We can help, with a care plan tailored to your needs.

Helping clients and your loved ones

Supporting the wider family and loved ones throughout your care journey is important.  To improve communications we can include people involved in your care in your “care-circle”.  This is a special feature of our state-of-the-art care management software and is only done at your request.  It allows your loved ones and your wider healthcare team to keep up to date with your care and gives them peace of mind about your progress. Our office-based care delivery team form part of your wider support team.  They are available to answer your questions and they will be checking in with you regularly.

What are Mobility & Frailty issues?

You might need mobility and frailty care for a number of reasons – such as recovering from a fall or accident, a muscle, joint or neurological condition or simply older age. Perhaps you need support coming home from hospital or would like additional care at home to help you feel safe and comfortable. Our mobility and frailty care will support you to remain as active as you are able – encouraging gentle movement and independence so you can live the life you love.  We will ensure you are comfortable, capable and safe.  Driving you to appointments, helping you wash and dress, keeping your house tidy and preparing meals are just some of the ways we can support you.
