Through our partnership with Oving Clinic, we can help you or your loved one access an experienced private GP for a comprehensive health review, either in the comfort of your own home or at the clinic.
It is an opportunity to take a little more time, look at the whole picture and talk through any concerns you or your family may have.

Our Home Health Review has been created for people who have recently returned home from hospital, are starting a new package of care, or would simply benefit from some additional medical reassurance.
An experienced GP will spend around 40 minutes getting to know the person’s current health, medication, existing conditions and any concerns that have arisen.
The appointment can take place at home, where things often feel more relaxed and familiar, or at Oving Clinic.
Medical assessments are provided independently by experienced GPs from Oving Clinic, with Vitale Care helping to make the process feel as straightforward and joined up as possible.
Leaving hospital or starting care at home often comes with a lot of information to take in.
There may be new medicines, follow-up appointments, changes to routines and questions that only become apparent once someone is back home.
That is where working together can make a difference.
Your Vitale Care team continues to provide the familiar, compassionate day-to-day support you know, while an Oving Clinic GP can provide the clinical assessment and medical guidance.
It means you, your family and your Care Professionals can feel more informed about what is happening and clearer about any next steps.
Most importantly, the focus remains on helping you feel safe, comfortable and confident at home, with the right people around you when you need them.
We also use GP Connect, where appropriate, to help authorised team members securely access relevant GP information for safer, better-informed care.
No, all invoicing and payments are managed through our head office. You’ll receive an invoice every two weeks, and instructions on how to pay are included with it.
We will always do our best to arrange support promptly, often within 24 hours. The first step is a conversation about the person’s needs, followed by a care assessment so we can create the right care plan and find a suitable Care Professional.
We take time to understand the person, not just the support they need. We consider their personality, interests, routines and preferences, as well as the experience required. Wherever possible, we will arrange an introduction before care begins and provide familiar, consistent Care Professionals.
Yes. Care needs can change gradually or quite suddenly, so we review each care plan and adapt the support when needed. This might mean changing the length or frequency of visits or considering overnight or live-in care.
Yes. We welcome input from family members and the people who know someone best, provided the person receiving care is happy for them to be involved. Their knowledge of routines, preferences and the little things that bring comfort can help us provide truly personalised support.
Yes. With the appropriate consent, we can work alongside GPs, district nurses, therapists, hospice teams and other professionals involved in someone’s care. This joined-up approach helps day-to-day support at home complement any clinical care or rehabilitation already in place.
We aim to provide a small, consistent team of familiar Care Professionals wherever possible. Continuity helps build trust and allows our team to understand the person’s routines, preferences and changing needs.

The initial comprehensive appointment lasts approximately 40 minutes, giving the GP time to understand the person’s health, medication and any concerns properly.
The GP can look through available discharge information, review current medication, discuss existing health conditions and listen to any concerns you or your family may have.
They can then talk through any recommendations or next steps with you.
Yes. The people around us often notice small changes that can be helpful to understand.
With your knowledge and in line with appropriate consent and confidentiality, family members or Care Professionals can share relevant observations with the GP.
Where appropriate, and with your consent, the Oving Clinic GP can liaise with your NHS GP, specialist or other healthcare professionals involved in your care.
Yes, where the GP feels these are clinically appropriate.
Oving Clinic may be able to arrange private prescriptions, blood tests, ECGs, imaging or onward specialist referrals. Additional charges may apply and these will be explained to you.
If a follow-up GP appointment would be helpful, this can be arranged either at home or at Oving Clinic.
Follow-up appointments are booked and charged separately.
The cost depends on whether the appointment takes place at Oving Clinic or in your own home. For home visits, the cost may also vary depending on your distance from the clinic.
